Information needed before coming to camp

This form must be completed just before the start of the Program. As it contains basic health information, in order to properly attend to the Participant, please complete the data in as much detail as possible. During admission to the center, you will also be asked for (1) the health card or private assistance insurance valid in Spanish territory (the agreement with Andorra is admitted), (2) the vaccination sheet, (3) in the case of transfers, the identity document to travel, together with the flight, and (4) in the case of foreign Participants, the valid passport. These documents will be returned at the end of the Program. The data collected will only be used for personal and health care.

This document includes the General Conditions initially accepted during registration, indicating those applicable by the start of the Program.

These are the documents necessary to continue with the form: Photocopy or Image of the Vaccine Book and Photocopy or Image of the Health Card.
